MONITORING AND FEEDBACK FORM
If you have been awarded funds from Screen South, you will need to fill in a Post Completion Monitoring Form as one of the requirements to secure the final instalment of your award. Completed forms enable Screen South to provide accurate feedback to its funding partners as well as to measure the impact of its awards on the region.
The form is available to download or we can also send you a copy in the post. Please complete the form as fully as possible. If you have any queries about the completion of the monitoring form, please contact Screen South.
